iptables -f will delete all the rules in chain or all chains.
It should work after deleting the rules.

It seems that you have added incorrect storage on the virtualizor panel.
"/dev/md3" is a Device and not a file.

If you want to add file based storage you will need to create a VG with /dev/md3 and mount it on the directory you would like to create VPSs in and if you want to use LVM based storage then simply add the VG in virtualizor panel.

You can follow our guide to add the storage on  virtualizor panel.
